Welcome.
The Charitable Fund Award from Commonwealth Community Trust (CCT) provides funds for equipment and/ or services to individuals with disabilities who demonstrate a financial need and reside in Virginia. The amount of an award per individual is up to $1,000.
The application must be completed and verified by a representative from a nonprofit organization or public agency in Virginia that serves people with special needs.
Returning Users:
If you are a returning user, please login under “Sign In” using the login credentials that you used for last year's application. If you forgot your password, select "Forgot your password?” to retrieve your login information. Once you've successfully signed in, you will be able to submit applications on behalf of the individuals you serve.
New Users:
If you are new, please select "Sign Up" under "Need an Account" to complete your registration as an applying organization's representative. Once you've successfully registered, you will be able to submit applications on behalf of the individuals you serve.
Before submitting an application, the following requirements must be met:
The applicant lives in Virginia and has a disability that meets the SSA definition of disabled. Disability for adults is defined as the inability to engage in “substantial gainful activity.” There must be a physical or mental impairment that can be expected to last at least 1 year. A child must have a physical or mental condition that very seriously limits his or her activities and is expected to last at least 1 year.
The applicant meets Federal poverty guidelines and/or receives SSI benefits. For the HHS Poverty Guidelines, go to http://aspe.hhs.gov/poverty-guidelines. For a minor, eligibility would be based on the family's income.
The application must be completed and verified by a representative from a nonprofit organization or public agency that serves people with disabilities.
The funds being requested are for the purchase of either equipment or services (not paid for by Medicaid, private insurance, or another source of funds) that will increase the applicant's ability to perform activities of daily living, or to perceive, control or communicate within the environment in which he or she lives.
The application includes a price quote or estimate from the vendor.
Other sources of funds have been exhausted, denied or are unavailable.
IMPORTANT
This program is not intended to pay off a applicant's debts, to fund services that are normally provided at no cost to the applicant, to pay for utility bills, or to provide a general source of income to the applicant or to the applying organization/agency.
